Our psychiatric mental health nurse practitioners provide outpatient evaluations and medication management. With appropriate authorization, we coordinate with the professionals already supporting your patient.
For established psychiatric patients without a PCP, primary-care support is available alongside their psychiatric care.

Parent or legal guardian consent is required. Please confirm that the patient and guardian have been informed of the referral and can participate in intake.
Privacy note
Referral documents are stored only long enough for our team to receive them via secure email, then deleted automatically after 7 days. They are never visible to the public or kept in the website database.
Outpatient referrals only
This form is not monitored for emergencies. For immediate danger, call 911. For suicidal thoughts or a mental health crisis, call or text 988. Do not submit a crisis request here.
