We use your correct name and pronouns regardless of what appears on your insurance card, and we record them in your chart. Teens and adults are treated with the same respect. Tell us at intake how you want to be addressed and how we should contact you.

We will help you complete the form if you need assistance, and we will not retaliate against you.
