Website Privacy Policy

This policy explains what happens to information you enter on sanevihs.com. Health information you share with us as a patient is separately governed by our Notice of Privacy Practices. We designed this website to collect as little as possible and to keep nothing it does not need.

1. Zero-retention design

Information submitted through our intake request form, self-check questionnaires, and professional referral form is transmitted over encrypted connections (TLS) directly to our secure practice mailbox and is not stored in a website database or analytics system. We do not keep a copy of your answers on this site.

  • Intake requests are emailed to our clinical intake team and discarded from the web application immediately after delivery.
  • Self-check results are calculated in your browser session, shown to you on screen, and emailed only if you ask us to send them. Scores are educational screening tools, not diagnoses.
  • Referral documents are uploaded to private, encrypted storage that only our practice can access through short-lived secure links, and are automatically deleted after seven days once retrieved into the patient record.

2. Information we collect

  • Information you provide: name, date of birth, contact details, parent/guardian contact details for patients under 18, reason for seeking care, insurance information, questionnaire answers, and any documents a referring professional uploads.
  • Technical information: our hosting provider processes standard server logs (IP address, timestamp, requested page) for security, abuse prevention, and rate limiting. These logs are not linked to your health information and are retained only briefly.
  • Cookies: this site uses only essential cookies required for the pages to function. We do not run advertising trackers, sell data, or share information with data brokers, and we do not place third-party marketing pixels on pages where health information is entered.

3. How we use information

Only to respond to your request: to confirm clinical fit, verify insurance, contact you or your guardian about scheduling, coordinate a referral, and meet our legal and professional obligations. We do not use website submissions for marketing, and we do not send promotional email.

4. Email is not fully secure

Standard email is not a secure channel. Please do not include detailed clinical history, diagnoses, or sensitive identifiers in ordinary email. Use the referral upload form, call us at (708) 948-7039, or use the patient portal for anything sensitive. By submitting a form or emailing us, you consent to our replying by the contact method you provided.

5. Minors

This site is intended for adults and for teens age 13 and older who are seeking care with a parent or guardian involved. We do not knowingly collect information from children under 13, consistent with the Children's Online Privacy Protection Act (COPPA). If you believe a child under 13 submitted information, contact docs@sanevihs.com and we will delete it.

6. Service providers

We use a small number of vendors for hosting, encrypted file storage, and email delivery. Vendors that may handle protected health information operate under HIPAA business associate agreements and are limited to the minimum information necessary to perform their service.

7. Security and breach notification

We use TLS encryption in transit, encryption at rest for uploaded documents, role-restricted access, automated abuse and rate-limit protection, and routine review of access. No system is perfectly secure. If a breach affecting your information occurs, we will notify you as required by HIPAA and the Illinois Personal Information Protection Act (815 ILCS 530).

8. Your choices and rights

You may request access to, correction of, or deletion of information you sent us through this site, and you may ask us to stop contacting you, by emailing docs@sanevihs.com or calling (708) 948-7039. Patients have additional rights described in our Notice of Privacy Practices. We honor Global Privacy Control signals where applicable and do not sell or share personal information as those terms are defined under state privacy laws.

9. Changes

We will post any changes to this policy on this page with a new effective date. Material changes affecting how we handle health information will also be reflected in our Notice of Privacy Practices.
